Required Medical Form

All incoming students must complete the Required Medical Form online prior to arriving at OWU. You won't be able to attend classes until the form and additional documentation have been completed, submitted, and processed by the Health Center staff. Immunization Policy

Incoming students must also submit to the Student Health Center proof of vaccines from a health care provider. Our Immunization Record Form (PDF) may be printed and taken to your medical provider to complete, or you may submit an official immunization record from your medical provider.

Athletic Physical Forms

All athletes are required to have a physical with an OWU physician prior to participation in sports each academic year. There is no charge for the physical if completed on designated OWU physical days. Additional information and physical forms are available at the OWU Athletic Training website.
