This is a health services checklist for all incoming students. Please read this page carefully and follow the steps listed. If you have a question not answered here, see our General Information page or FAQ page, or contact the office.
All portions of this checklist are due June 12 2024, for all Fall 2024 incoming students.
REQUIRED (ALL STUDENTS)
- Immunization Record Form (PDF): An official immunization record must be submitted to the Health Center. You can print a form to take to your health care provider. From the link above, submit an official record from your health care provider's office or from your high school. See the Health Service Forms page under Immunization Policy for additional information.
May be Required Based on Submitted Info
- Tuberculosis (TB) Form (PDF): If you are instructed on the above Medical Form to complete a Tuberculosis (TB) test, you must submit documentation of the test via fax, email, or mail. A form to take to your health care provider can be printed from the above link or the Health Service Forms page.
- Chronic/Ongoing Medical Conditions: If you are an incoming student with medical issues and have further documentation needed for care, please contact Student Health Center at 740-368-3160 or health@owu.edu.
